The plan and investment information in this section was developed to help you meet your participant disclosure responsibilities. It will be posted to your participant website for ongoing availability.

If Voya is the exclusive provider to your plan, the disclosure will be included in information that you should provide to newly enrolling participants and will be posted to your participants' website for ongoing availability. You should provide this information to eligible non-participating employees annually and as part of upcoming enrollment announcements.

If there is more than one product provider for your plan, you should provide this information annually in conjunction with the reports prepared by the other product providers to all eligible, non-participating and participating employees. This information must also be provided to new hires. This disclosure will be posted to your participants' website for ongoing availability.
