Annual General Plan Administrative Fees

Please input any general plan-level administrative fees that will be deducted from participant accounts, on either a dollar or percent basis, and indicate the appropriate deduction method. Note that fees input here will be displayed on the plans’ required participant disclosure reports. Please also note that participants are required to be provided with 30 - 90 days advance written notice of any fee change and that Voya will assume that such notice has been provided, where required, prior to your submission of this form.

Please note that all fees documented through this tool are strictly for reporting purposes only and will be displayed on the next Participant Disclosure for the plan listed. Data reported through this tool is not used to actually deduct fees against participant accounts.

If there is a fee type that may be charged to participant accounts, but the actual amount is unknown at this time, please check the Unknown Amount box and leave the dollar and percent fields blank. You can also provide an estimated amount, by providing the dollars or percent and checking the Estimated Amount box. If you previously saved a fee amount but wish to remove it, you can check off the Delete Fee box. There is no need to remove any of the information previously input.

To input a percent, enter it in the xxx.xxx format. For example, 1.5 percent would be entered as 1.50. To convert a basis point to a percent you would divide it by 100. For example, 25.5 basis points would be 25.5/100 = 0.255. The data entered on this screen will be displayed on the Participant Fee Disclosure in the format that it is entered.

The "Deduction Method" column describes how each fee is allocated among participants. "Pro-rata" means that participants each pay a portion of a plan-level fee amount based on their share of plan assets. "Per capita" means all participants pay an equal share of a larger plan-level fee amount (i.e. plan-level amount is divided by the number of participants). "Per Participant" means that the same fee amount is assessed against all participants (e.g. $15/participant, etc.).

Please take special care to ensure correct input of the service provider fees. Voya shall not be responsible for the accuracy of any such fee information that appears on the disclosure.
