To get started with setting mail delivery preferences for all or the selected plans below, complete the following and click CONTINUE.

1. Set your mail delivery preferences

Online Choose an alert preference: Email and Web Site Web Site Only U.S. Mail	Please enter your email address in the spaces provided. If you’ve chosen e-mail as your alert preference, you will receive notification at this address when new documents you would have received by mail are posted. 2. The above mail delivery preferences will be applied to plans you select below. Plans shaded in orange have already been set up. To apply to ALL plan, check the “Select All” box in the first column.
