      To upload Completed Benefit Election Paperwork or Other documents, please choose the appropriate tab.

      Official Seals must be visible in the uploaded file. If the document (such as the Spousal Consent form) has a raised seal, the seal should be made visible by using a pencil to shade over the raised portion of the seal prior to scanning and uploading.

      You may upload multiple documents if needed. You can also upload additional files at another time if you don't have everything ready now. After uploading all documents, you MUST click Submit. If you do not click submit, your documents will not be processed.

      Once submitted, we will notify you of receipt and your paperwork will be reviewed. If we require any additional information we will notify you.

      In order to complete the payment election process, you will need the following information:

      • Your beneficiary information including address, social security number and date of birth
      • Your rollover institution information if you are eligible to rollover your benefit
      • Any information needed to complete your tax withholding election
      • Your bank information to have your payments direct deposited
